Transfer of records
If you are moving or transferring to another physician, we are happy to provide a copy of your child’s medical record to the receiving physician. You may download or pick up a form to request this. Once we receive the signed request, we will provide one copy to the physician you are transferring to, free of charge.

If you would like a copy of your child’s medical record for yourself, or if you would like additional copies of the chart, an administrative fee of $15 plus 20 cents per page will be charged. In addition, postage will be charged if the chart has to be mailed.
